Five steps. The more complete the paperwork, the faster an attorney can tell you where you actually stand. Submitting this does not create an attorney-client relationship and commits you to nothing.
Start with the entity that signed the advance agreements.
The first owner is required. Add the others only if they signed a guaranty or hold an ownership stake.
List every open advance, including the ones taken to cover other advances. One row each.
| Funder Name | Original Amount | Current Balance | Regular Payment | Payment Frequency | Ending Date | Remove row |

This is the step that decides how fast the review moves. Send everything you have; we will tell you what is missing.
Check what you have entered, then sign to send it to the firm.
